The first eight pages of the prefatory material are bound out of order (A1, A3, A2, A4), so that the original page order is 1, 2, 5, 6, 3, 4, 7, 8. In addition, the first eight pages of the main text are misbound in the same way (B1, B3, B2, B4), so that the original page order is 1, 2, 5, 6, 3, 4, 7, 8. These errors have been corrected in the transcription so that the text reads in order.
